Family Connects is a free, nurse home visiting program available to all families in Lancaster County who have recently given birth or welcomed a newborn into the home. Specially trained nurses can provide 1-3 visits offering postpartum and newborn support during the first weeks after birth.
Nurse Visit Includes:
- Newborn weight check and assessment
- Maternal health check
- Breastfeeding and feeding support
- Postpartum mental health screening and support
- Newborn care education
- Safety resources
